About Facing Forward to End Homelessness

Facing Forward to End Homelessness helps families and individuals exit homelessness by providing stable housing and support services to improve health, build life skills, and increase self-sufficiency. Residents in our program have experienced chronic homelessness and are living with a disabling condition, compounding the challenges of reaching stability on their own. Every resident is paired with a case manager, and together they develop an individualized plan to achieve goals around housing stability, health improvement and self-sufficiency through a harm-reduction, trauma-informed, and strengths-based approach. Last year, we helped end homelessness for over 725 people in Chicago.
